I hereby express my desire to become a member of the
VISAYAN AREA NEIGHBORHOOD ASSOCIATION, Inc. (VANA) and pledge to follow all
policies and regulations set by the Association, its officers, and governing committees.
Ako ay nagnanais na maging kasapi ng
VISAYAN AREA NEIGHBORHOOD ASSOCIATION, Inc. (VANA) at nakalaang sumunod sa mga
patakaran at regulasyon na nasasaad sa batas ng samahan at sa mga kinauukulan, buong pagkakalahatang
samahan at sa Lupon ng Patnugutan.
I agree to comply with the following:
- To pay the Membership Fee and Monthly Due (Magbabayad ng Membership Fee at Butaw)
- To contribute to the Mortuary Fund (Damay Fund)
- To follow all agreements made during the General Assembly (Sumunod sa lahat ng napagkasunduan ng Pangkalahatang Asembleya)
Should I move out or terminate my membership in VANA, I understand that any contributions made cannot be refunded.
All information stated herein is given voluntarily and truthfully to the best of my knowledge.
Kung sakaling mangungupahan at umalis na sa nasasakupan ng VANA ay hindi pwedeng ibalik (i-refund) ang anumang naibahagi sa Samahan.
Ang lahat ng nakasaad ay kusang loob kong ibinahagi nang walang kasinungalingan at sa abot ng aking makakaya.
